Enhanced Care Management – Agape Villages

Agape Villages is now participating in Enhanced Care Management (ECM) in various counties. ECM is a new service offered by Medi-Cal. Through ECM, Agape is offering help at no cost to those who are enrolled as Medi-Cal members and are facing life and health challenges. Once it is determined that you qualify, Agape Villages will help with challenges such as:

Mental, physical, and behavioral health issues.
Locate specialists and make appointments.
Food insecurities, unstable housing, transportation challenges.
Employment and education assistance.
Interactions with law enforcement or crisis intervention due to mental health and substance use.
New moms are transitioning home from hospitalization.
Follow-up care after leaving the hospital.
